Patient Ambassador
Patient Ambassadors work in partnership with the medical practice in which they are based to support patients, particularly those individuals who are identified as ‘harder to reach and vulnerable’, in accessing healthcare appropriately.
This service delivers non-clinical interventions focusing on the social component of care. This vital personalised care role works restoratively, engaging patients using shared decision- making to make a full assessment of their non-medical needs. Through working closely with patients the patient ambassadors are able to signpost appropriately and make onward referrals to specialist local services, building resiliency and capacity within the local community. This service also helps to improve appointment attendance within the surgeries as well as supporting GPs with QOF outcomes.

Being You Leeds
Being You Leeds is a partnership of third sector organisations in and around the city. This partnership is made up of: Touchstone, Barca, Hamara, Health for All, Holbeck Together, LS14 Trust, Women’s Health Matters, Community Links and Humans Being. The project provides groups, activities, training and awareness sessions to promote community wellbeing across Leeds.
Being You Leeds promotes positive wellbeing with people in Leeds in 3 ways:
- Providing groups and activities focusing on wellbeing. These fun, welcoming groups run 7 days a week, including weekends, and are based around activities chosen by group members.
- Providing training and awareness raising sessions to staff and volunteers working in community organisations and to members of the general public in Leeds.
- Tackling mental health stigma and discrimination through activities, training, campaigns and strategic partnerships.
Groups and Activities at Hamara
Chess & Games Group
This group takes place every Thursday between 1-3pm, here at the Hamara Healthy Living Centre. It is a fun and relaxed all ability group for adults to enjoy a sociable board game or two! Refreshments are provided. For more information about the group please contact Robert by calling: 07484 515529.
